Snowline Hospice

- Provide medical care to patients in hospice allowing families' caregivers to attend to their health and wellness needs.
- Provides 42 families with four hours a day of paid care during the last two weeks of life. This totals to 1,680 hours of care.
- Will allow for 42 families to keep their loved ones at home who are on a low income budget by providing the additional caregiver support.
- Families' financial needs will be accessed to determine need and eligibility for the program
- Each qualified family will be responsible for interviewing and hiring their private duty caregiver. Snowline Hospice will provide a list of local caregivers and will, upon receipt from the caregiver and verification of services, cover the cost of the caregiver to individuals who qualify under this grant award.

- As indicated with the older adult survey, many older adults lack the skill and communication needed for navigation of the health care system. The grant will provide 12 hours per week of Palliative care nurse time.
- Provide information about the disease in question, its progression and treatment options.
- Gather contact data from charts to make quarterly chart reviews with clients and quantify the types of information needed and the knowledge level of clients.
- Develop and provide educational workshops during the funding year: two for the general public and one for health care professionals, including first responders.
- Create a “tips for health” newsletter.
